An initial consultation will determine your fitness requirements and help you to set appropriate goals.

Your personal aims and lifestyle information will then be used to design an appropriate training program.

The following when signed will constitute the full agreement between Chloe Brant (herein referred to as “trainer’) and client.

The trainer will act as a Personal Trainer in the development of an exercise programme during the agreed sessions.

In the consideration of the service, the client shall pay in advance according to the session rates charged by the trainer. Refunds are at the trainer’s discretion.

Personal Injury Waiver and Photography/Media Consent

I understand that my participation in exercise programs, classes or events and any related activity conducted by Chloe and Chloe Brant Fitness is voluntary and at my own risk. I am aware that exercise can be physically stressful and, in certain instances, even result in injury or cause of death. The levels of exercise that I will perform will be at my own pace, based upon my cardiorespiratory fitness, muscular strength, movement capabilities and endurance levels.

I hear by state that I will inform Chloe and Chloe Brant Fitness of any symptoms (E.G. fatigue, shortness of breath, chest discomfort, any pain/discomfort/concern for my safety) during my participation in exercise. If I have high blood pressure, diabetes, a heart condition, any existing injury, recent surgery or if I am taking any prescribed medications that could affect my performance, I will inform Chloe prior to participating in any exercise.

Clients agreement with Chloe Brant Fitness:

There will be no charge incurred for any sessions cancelled for any reason if at least 24 hours notice is given

If cancelling a session within 24 hours of the arranged time for ANY reason then a full charge will apply.

If the session can be rearranged for a convenient time for both client and trainer on the SAME day then charges will not apply.

Additionally the client agrees to give one calendar month’s notice of any termination or adjustment to the session plan they have signed up for.

The Chloe Brant Fitness agreement with client:

There is no liability to the trainer if they cancel an agreed session with at least 24 hours notice.

If the trainer cancels a session within 24 hours of the arranged time for ANY reason then they must give the next booked session completely free of charge.

If the session can be rearranged for a convenient time for both client and trainer on the SAME day then charges will not apply.

Limitations of Liability

Chloe and Chloe Brant Fitness, will not be liable for any injuries or damage arising from participation in training to the undersigned.

I understand that all financial transactions between me and Chloe Brant Fitness are direct and in no way associated with the facility at which I am trained.

It is agreed that Chloe, shall not be responsible or liable to the undersigned for articles lost or stolen whilst training with Chloe.

By taking part in training with Chloe Brant Fitness (including but not limited to personal training, group training or training events) I agree that full rights to the use of images resulting from photography/video filming during my training, and any reproductions or adaptations of the images can be used for fundraising, publicity or other purposes to help achieve the aims and objectives of Chloe Brant Fitness. This might include (but is not limited to), the right to use them in printed and online publicity, social media, press releases and funding applications.

Declaration of understanding

I, the client, agree that I fully understand and agree to abide by the above rules and regulations.

I understand that I should inform the trainer of any medical condition and that I may be advised to visit my doctor prior to commencing the sessions I acknowledge that I have either had a physical examination and have been given my doctors permission to participate, or that I have decided to participate in the activity and/or use the equipment or machinery without the approval of my doctor and do hereby assume all responsibility for my participation and activities.

I release my trainer Chloe Brant from any liability for personal injury or other damage I may sustain whilst engaging in any exercise programme suggested by my trainer.

Thank you for your cooperation, I hope you enjoy your Personal Training sessions. I acknowledge receipt of this agreement and confirm acceptance of the terms and conditions contained therein.
